NMHC is a coalition of health care providers in northern Michigan providing primary and acute care; mental health care; substance use treatment; dental care; elder services; and public health.
We hold the vision that we, as a community of health care providers, collaborate to provide a range of health care services to people of all ages in which we care for the whole patient and manage multiple, interrelated and chronic health problems. We have agreements and processes in place enabling coordination of care between our different provider systems as agreed upon by persons served.
Furthermore we agree to align our clinical and financial incentives to meet the triple aim of improved quality, patient experience and reduced costs. This would entail projects such as the development of mutally accessible electronic health record.
